OBJECTIVES/GOALS: Supported by the State of Alabama, the Alabama Genomic Health Initiative (AGHI) is aimed at preventing and treating common conditions with a genetic basis. This joint UAB Medicine-HudsonAlpha Institute for Biotechnology effort provides genomic testing, interpretation, and counseling free of charge to residents in each of Alabama’s 67 counties. METHODS/STUDY POPULATION: Launched in 2017, as a state-wide population cohort, AGHI (1.0) enrolled 6,331 Alabamians and returned individual risk of disease(s) related to the ACMG SF v2.0 medically actionable genes. In 2021, the cohort was expanded to include a primary care cohort. AGHI (2.0) has enrolled 750 primary care patients, returning individual risk of disease(s) related to the ACMG SF v3.1 gene list and pre-emptive pharmacogenetics (PGx) to guide medication therapy. Genotyping is done on the Illumina Global Diversity Array with Sanger sequencing to confirm likely pathogenic / pathogenic variants in medically actionable genes and CYP2D6 copy number variants using Taqman assays, resulting in a CLIA-grade report. Disease risk results are returned by genetic counselors and Pharmacogenetics results are returned by Pharmacists. RESULTS/ANTICIPATED RESULTS: We have engaged a statewide community (>7000 participants), returning 94 disease risk genetic reports and 500 PGx reports. Disease risk reports include increased predisposition to cancers (n=38), cardiac diseases (n=33), metabolic (n=12), other (n=11). 100% of participants harbor an actionable PGx variant, 70% are on medication with PGx guidance, 48% harbor PGx variants and are taking medications affected. In 10% of participants, pharmacists sent an active alert to the provider to consider/ recommend alternative medication. Most commonly impacted medications included antidepressants, NSAIDS, proton-pump inhibitors and tramadol. To enable the EMR integration of genomic information, we have developed an automated transfer of reports into the EMR with Genetics Reports and PGx reports viewable in Cerner. DISCUSSION/SIGNIFICANCE: We share our experience on pre-emptive implementation of genetic risk and pharmacogenetic actionability at a population and clinic level. Both patients and providers are actively engaged, providing feedback to refine the return of results. Real time alerts with guidance at the time of prescription are needed to ensure future actionability and value.

Background: The challenges of chronic pain management, and resulting poorer outcomes, in workers’ compensation (WCB) patients has been well established. Spinal cord stimulation (SCS) has been used for the management of low back and radicular neuropathic pain with varying effectiveness and it’s efficacy in the WCB population has been challenged. We sought to examine our experience using SCS in WCB compared to non WCB patients. Methods: A retrospective analysis of 71 WCB patients assessed and treated at the St Pauls Hospital neuromodulation program between 2016-2021 was performed. This group was compared to a cohort on non WCB patients in terms of the likelhood of being offered a trial, proceeding with trial if offered, and the likelhood of a successful trial proceeding to implant. Results: Compared to non WCB, the WCB patients were more likely to be offered a trial (86% vs 77%) and more likely to proceed with a trial if offered (82% vs 71%). Trial to implant ratios were similar in both WCB and non WCB patients (78% vs 77%). Conclusions: WCB patients were more likely to be offered a SCS trial and more likely to accept if offered, compared to non-WCB patients. However, both groups were similar in trial to implant probability.

Background: Refractory angina is defined as a chronic condition characterized by the presence of angina due to coronary insufficiency which cannot be controlled by a combination of medical therapy, angioplasty and coronary bypass surgery. Prevalence in the United States is estimated to be between 300,000-900,000. Spinal cord stimulation for refractory angina pain relief was first described in 1987 but is still not widely used in North America. We report our experience with this treatment. Methods: A retrospective review of patients referred to the St Pauls Hospital neuromodulation program for consideration of SCS for refractory angina was conducted. Patients underwent implantation using a either a two stage approach (percutaneous or permanent lead trial followed by full system implantation) or full system implantation. Results: Bewtween 2004-2020 36 patients underwent full system implantation (2 patients failed the trial and were not implanted). Of the 36 patients undergoing full system implantation, 33 (92%) experienced significant reduction of angina, increased exercise tolerance and/or medication reduction and were considered successful implantation. Most common lead placement location was at C7 T1. Conclusions: Spinal cord stimulation is an effective therapy for patients suffering from crippling angina pain despite medical optimization.

Background: The dorsal root ganglion (DRG) has been established as an important structure in the development and modulation of chonic neuropathic pain and has demonstrated superiority over spinal cord stimulation in the management of challenging neuropathic pain conditions, including complex regional pain syndrome and chronic post-spine surgery neuropathic pain. DRG has only been available in Canada for patient application since January 2020. The St Pauls Hospital Neuromodulation Program was one of the first Canadian centers to offer this procedure. Methods: We reviewed our early experience with DRG therapy in 10 patients. Patient-reported outcome measures were collected pre-trial, post-trial, and post-device implantation, to determine the efficacy of DRG. We hypothesized that DRG stimulation would demonstrate a meaningful change in PROMIS-29 domains and at least 50% improvement in pain intensity at 8 wks. Results: All patients demonstrated a > 5 point change in T scores in PROMIS-29 domains suggesting a meaningful benefit. Patients also demonstrated a percentage pain improvement at 8 wks of 64 % based on a numerical rating scale. No major complications were observed. Conclusions: DRG stimulation is a safe and effective treatment option for neuropathic pain.
Implementation of the Targeted Assessment for Prevention Strategy in a healthcare system to reduce Clostridioides difficile infection rates

Background:
Prevention of Clostridioides difficile infection (CDI) is a national priority and may be facilitated by deployment of the Targeted Assessment for Prevention (TAP) Strategy, a quality improvement framework providing a focused approach to infection prevention. This article describes the process and outcomes of TAP Strategy implementation for CDI prevention in a healthcare system.
Methods:Hospital A was identified based on CDI surveillance data indicating an excess burden of infections above the national goal; hospitals B and C participated as part of systemwide deployment. TAP facility assessments were administered to staff to identify infection control gaps and inform CDI prevention interventions. Retrospective analysis was performed using negative-binomial, interrupted time series (ITS) regression to assess overall effect of targeted CDI prevention efforts. Analysis included hospital-onset, laboratory-identified C. difficile event data for 18 months before and after implementation of the TAP facility assessments.
Results:The systemwide monthly CDI rate significantly decreased at the intervention (β2, −44%; P = .017), and the postintervention CDI rate trend showed a sustained decrease (β1 + β3; −12% per month; P = .008). At an individual hospital level, the CDI rate trend significantly decreased in the postintervention period at hospital A only (β1 + β3, −26% per month; P = .003).
Conclusions:This project demonstrates TAP Strategy implementation in a healthcare system, yielding significant decrease in the laboratory-identified C. difficile rate trend in the postintervention period at the system level and in hospital A. This project highlights the potential benefit of directing prevention efforts to facilities with the highest burden of excess infections to more efficiently reduce CDI rates.
Determining the economic value of daily dry matter intake and associated methane emissions in dairy cattle

Feed represents a substantial proportion of production costs in the dairy industry and is a useful target for improving overall system efficiency and sustainability. The objective of this study was to develop methodology to estimate the economic value for a feed efficiency trait and the associated methane production relevant to Canada. The approach quantifies the level of economic savings achieved by selecting animals that convert consumed feed into product while minimizing the feed energy used for inefficient metabolism, maintenance and digestion. We define a selection criterion trait called Feed Performance (FP) as a 1 kg increase in more efficiently used feed in a first parity lactating cow. The impact of a change in this trait on the total lifetime value of more efficiently used feed via correlated selection responses in other life stages is then quantified. The resulting improved conversion of feed was also applied to determine the resulting reduction in output of emissions (and their relative value based on a national emissions value) under an assumption of constant methane yield, where methane yield is defined as kg methane/kg dry matter intake (DMI). Overall, increasing the FP estimated breeding value by one unit (i.e. 1 kg of more efficiently converted DMI during the cow’s first lactation) translates to a total lifetime saving of 3.23 kg in DMI and 0.055 kg in methane with the economic values of CAD $0.82 and CAD $0.07, respectively. Therefore, the estimated total economic value for FP is CAD $0.89/unit. The proposed model is robust and could also be applied to determine the economic value for feed efficiency traits within a selection index in other production systems and countries.

The objective of this study was to assess the prevalence of Ureaplasma urealyticum and Mycoplasma hominis infections and to investigate associations between their presence in the lower female genital tract and lifestyle characteristics. The study was performed on a population of 3115 women, comparing the demographic and behavioural characteristics of 872 women with U. urealyticum infection and 142 women with M. hominis with uninfected women, using univariate and multiple logistic regression analysis. The prevalence of infection with U. urealyticum was 28% and M. hominis was 4·6%. In multivariate logistic regression analysis, intrauterine device, number of sexual partners and age (<35 years) were significantly associated with U. urealyticum while previous induced abortion, condom use and young age at first intercourse (<16 years) were associated with M. hominis infection. U. urealyticum infection presents the same demographic and behavioural characteristics of a sexually transmitted disease. The unprotective role of condom use suggests a non-sexual mode of transmission of M. hominis infection.
Delayed cochlear implantation after surgical labyrinthectomy

Background:
Cochlear implantation has been used to rehabilitate profoundly deafened adults for more than 25 years. However, surgical labyrinthectomy is often considered a contraindication to cochlear implantation, especially if there is a significant delay between the two procedures. As the role of cochlear implantation continues to expand, this idea requires reconsideration.
Case report:A 59-year-old woman presented to our clinic after undergoing bilateral surgical labyrinthectomies for intractable Ménière's disease 21 years prior. Despite the significant time delay, she underwent cochlear implantation with a good audiological outcome and improved quality of life.
Conclusion:Changes to the cochlea and vestibule following surgical labyrinthectomy include cochlear ossification and obliteration of the vestibule. These issues have been thought to limit the potential for cochlear implantation, especially when there is a significant delay between the two procedures. However, delayed cochlear implantation, even decades after labyrinthectomy, remains a viable treatment option which can benefit selected patients.

Tracheostomy remains the primary method of treatment of acute airway obstruction due to malignant invasion and compression of the trachea. However, the development of tracheal stents has provided an alternative effective treatment modality. This case report and literature review highlights the benefits of intra-luminal stenting, including resolution of distressing obstructive symptoms, and subsequent improvement in quality of life.

Multiwavelength observations of polars are essential for developing the big picture of these systems, particularly to gain understanding of the relevant accretion-induced heating and cooling processes. Eclipsing polars are prime targets for such studies since different radiation processes can be disentangled by observations with high-time resolution. We present a preliminary combined analysis of space-based observations (XMM-Newton, ROSAT, HST) with ground-based high-speed photometry (MCCP, OPTIMA, ULTRACAM) of DP Leo, HU Aqr and UZ For. We determine the location and extent of different emission components and find secular and short-term changes in the accretion geometries. We find displaced optical and X-ray emission regions in DP Leo and HU Aqr as well as mini-bursts and accretion arcs of variable size in HU Aqr. We report marked changes in the X-ray eclipse length of UZ For between high and low states.

Inelastic neutron scattering (INS) results on the intermultiplet transitions J=0 → 1 and J=1 → 2 in optically opaque EuBa2Cu3O7 are reported. Whereas these multiplets are split by the crystalline electric field (CEF), their low J values are influenced to first order only by the second-order (J=1) and additional fourth-order (J=2) CEF parameters. and the spinorbit coupling parameter were obtained by fitting the splitting of the J=l multiplet and the energy separation between the J=0 and 1 multiplets. The J=0 → 1 splitting observed here is smaller than previously seen by optical spectroscopic studies on a variety of transparent, ionic compounds, necessitating fitting of the free-ion parameter. Additional spectroscopic information on the J=2 multiplet indicates that additional fitting of free ion parameters must be included to adequately model the observed low energy separation between the two lowest J-multiplets. Preliminary calculation on the Q-dependence of the CEF split J=0 → 1 transitions and the comparison with observations are presented.

The response of patients with major depressive illness to citalopram of amitriptyline was compared in a double-blind multi-centre trial. No differences in efficacy were observed, but citalopram had less hypnotic effect and a remarkably lower profile of side-effects.
